This is the first page of the standard two-page EKG / pacemaker report to be sent to physicians (of course all names, addresses, and patient number have been hidden here for privacy reasons). Note that this and all reports were printed on plain paper by a laser printer. All information was gathered dynamically from billing (name and address), history, and current-test database tables to create this report with only the most up-to-date patient and physician information.
